How May I Pay?

We accept payment by cash, bank or cashier’s check, debit card, VISA, Mastercard, American Express and Discover. CareCredit Financing is available. Call our office to find out which plans are available.

Do I Need A Referral?

No referral is required unless you have an HMO or managed care plan that requires insurance referral authorization from your primary care physician. It is your responsibility to initiate the referral from your primary care physician. If we have not received an authorization prior to your arrival at the office, we have a telephone available for you to call your primary care physician to obtain it. If you are unable to obtain a required insurance referral before your appointment, you will be rescheduled.

What If I Need Surgery?

If your physician recommends surgery, you will need to see the Surgery Coordinator. She will answer specific questions about the surgery scheduling process, discuss the paperwork and tests involved, and complete all pre-certification/authorization, if your insurance company requires it.

The Billing Department will request a pre-surgical deposit, the amount of which depends on your coverage and deductible amount. A cost estimate that shows your financial responsibility, based on the benefit levels and coverage of your insurance plan, will be explained by the Billing Department.

In the event that your surgery is cosmetic and is being done for aesthetic reasons, pre-payment of surgical fees will be required at least 14 days before the surgery date.

What If My Child Needs to See the Physician?

A parent or legal guardian must accompany patients who are minors on the patient’s first visit. This accompanying adult is responsible for payment of the account, according to the privacy policy outlined.

Which Plans Do You Contract With?

We participate in Medicare, Medicare HMO and Wisconsin Medicaid programs. We are members of the Bellin/ThedaCare Physician Partners, Prevea (Arise) HMO and PPO, Network Health Plan, United HealthCare, Humana, Health EOS, Multiplan/WPPN, Blue Cross/Blue Shield and many varied plans. Because policy plans vary by employer, please check with your plan administrator to see if we are listed as a participating provider. Please notify us if we are not in your insurance network so that we may contact them to make every effort to provide services to you. Not all insurance plans cover all services. If your insurance plan determines a service to be “non-covered,” you will be responsible for the complete charge. Payment is due upon receipt of a statement from our office.
